Bulk Bill Post-Assignment of Benefit Form (DB4)

A Bulk Bill Assignment of Benefit Form (DB4) is provided when a bulk bill claim has been submitted to the agency for processing, for the benefit amount to go to the health professional.

When submitting a Bulk-Bill claim to Medicare, CESoft will display to the user the Bulk Bill Post-Assignment of Benefit form (DB4):


If the patient has not already agreed to assign their Bulk Bill benefit to the provider, the user can use this as a chance to go through the bulk Bill Assignment of Benefits process to the patient and have them accept to their benefits being sent to the provider. Once either is achieved, clicking Assign Benefit and Submit will transmit the claim to Medicare. After this, the form can be found in the History tab on the invoice screen, and can be viewed/printed as needed by clicking the document icon on the rightmost side of the line the form is in:
