Pre-Op Questionnaire

Cutting Edge has a Pre-op Patient Questionnaire for Anaesthetic patients

1. Send a link to the Pre-Op Questionnaire to the patient via their email address on their Invoice screen:



2. The patient will receive the following to their email address:


3. The patient then needs to click on the link to see the Pre-Op Questionnaire form.

4. Once the questionnaire has been filled out, the patient needs to click on submit on the bottom of the form.

5. A notification is then sent to the CESoft user advising that the patient has completed the form. The date submitted will be recorded on the invoice screen, in the Procedure Details box in the bottom right section:



When the Pre-Op Questionnaire has been sent to the patient, the CESoft user can determine when the form was sent, when the user viewed the form and when it was submitted by looking in the "Procedure Details" box on the Invoice page:



6. To view the completed questionnaire - the Provider needs to click on Pre-Op button on the relevant invoice, this will open in a new window on their computer with the completed form.

If the provider wishes to get a copy of the questionnaire once completed by the patient, CESoft users can enable this by following these steps:
  1. Navigation Panel - Settings > General > Prompts > Set the Primary or Secondary email address to receive the completed questionnaire.

A patient may also print off their Pre-Op questionnaire and submit it via email or keep a copy by scrolling to the bottom of the Pre-Operative Questionnaire and clicking the "Print" button on the bottom of the screen:




27-05-26 - At this stage, the CESoft Pre-Op Questionnaire cannot be customised, however the CESoft team is aware that many users would like for it to be customisable and as such, that is a planned feature to be added in the future.
