MBP:  For an adjustment or rejection via Eclipse call MBP to get a CARS reference number and then email with batch header  https://www.medibank.com.au/content/dam/retail/providers/gap-cover/GapCover-batch-header.pdf

NIB: Fund does not accept manual forms. Call the fund. They will discuss the claim with you and issue a case number, then they will accept an email with this case number entered on the subject line of email
